Hiring nurses from abroad: cost, process and recognition
Employers who want to hire nurses from abroad have to manage three things in parallel: professional recognition as a nurse, a visa under the Skilled Immigration Act, and proof of German at B2 level. How long it takes and what it costs depends less on the source country than on how well these three tracks interlock.
The domestic labour market is exhausted. According to the Federal Statistical Office, around 86,600 foreign professional qualifications were recognised in 2025 – roughly 32,000 of them in nursing. More and more hospitals and care providers are therefore building their own international pipelines instead of competing for the same shrinking local pool.

The cost of hiring international nurses: five building blocks
There is no flat figure – total cost is made up of five building blocks and depends on source country, recognition pathway and language preparation. Under the employer-pays principle of the RAL quality mark "Faire Anwerbung Pflege Deutschland", the employer bears it in full; placement fees charged to candidates are ruled out.
Sourcing & selection

Language training to B2
Courses from A1 to B2 in the source country plus nursing-specific German, completed with a recognised exam (e.g. Goethe or telc). The biggest block of time and cost – and the most important one for later integration.
Recognition
Application to the competent state authority, equivalence check of the qualification, certified translations and – where gaps exist – an adaptation course or knowledge test.
Visa & relocation

Process and timeline
From selection to the first day of work usually takes 4 to 12 months. What matters is that recognition, language preparation and visa run in parallel rather than one after the other – then the timeline sits at the lower end. Recognition: the decisive step
Nursing is a regulated profession. Without professional recognition as a nurse, an international nurse may not work independently. The competent state authority checks the equivalence of the qualification; where substantial differences exist, an adaptation course or knowledge test follows.
In the regular procedure the authority generally decides within four months of receiving complete documents, in the fast-track skilled worker procedure within two months. Recognition can run in parallel with the visa procedure – that is where most time is saved.
Fast-track skilled worker procedure (Section 81a)
Employers can apply for the procedure at the foreigners authority with a power of attorney from the nurse. The authority bundles recognition and the Federal Employment Agency's approval (deadline: one week), the visa appointment is granted within three weeks and the visa is usually decided within a further three weeks. The fee is €411.
Language level: B2 with nursing-specific German
Practising as a nurse in Germany generally requires German at B2 level – including nursing-specific language. Candidates prepared for this in the source country arrive with a language level that lets integration in the team work from day one.
What changed in 2026
Since 1 January 2026 the Nursing Competence Act (BEEP) has given nurses expanded independent medical competencies – including recognised international nurses. Also since 1 January 2026, employers are obliged under Section 45c Residence Act to inform newly arrived skilled workers about fair-integration services.
